Easiest way I know of reproducing this is to give user1 incomplete references and running the suite, which will throw AccessLevel duplicate composite PK issues
For me it showed in SM/admin-access when I added roles to user1 before moving the roles.yml load above the user.yml load (they were both always included though - so I imagine this is an even bigger issue!)
I hope this is enough to investigate. If not, I'm happy to provide exact commands to reproduce. At the behest of @kalifg I am creating this issue and assigning it to him
