refactor(ui): add shared Radix Avatar component

Summary

Adds a shared Avatar UI component based on Radix Avatar primitives for consistent avatar rendering across the application.

Changes

• Add reusable Avatar component in apps/web/components/ui/avatar.tsx
• Follow the existing shadcn/ui component structure and project conventions
• Reuse the Radix primitives already available on main
• No functional changes to existing pages

Notes

• Rebased onto the latest main
• Preserves the shared UI foundation already merged
• No dependency or lockfile changes included in this PR

Validation

• ✅ pnpm turbo run lint
• ✅ pnpm turbo run check-types
• ✅ pnpm turbo run test
• ✅ pnpm turbo run build
