Improve throttle notice emails and admin summary - #592

Reword the notice and enforcement email templates (HTML and text) so a single extreme day reads naturally instead of the misleading "over the past several days" / "across 1 day, including X in a single day" phrasing.
Surface how many times an account has already been notified in the per-candidate summary line, so admins can see escalation history at a glance instead of only inferring it from the enforce/warn verb.
