ADR Boundary Slice

  • Canonical ledger: none. No programme is active (AGENTS.md); this is a standalone fix.
  • Slice: MCP proxy decision outcome classification for an allow with no obligation outcome.
  • Builder: Claude Code (claude/ branch).
  • Reviewers: pending. One non-building review is needed on the final head.
  • Final head SHA: aeea160501a13d5cdc6b53f0972f6ef68a62c12a (macOS arm64, builder worktree).
  • ADR invariants affected: none. Policy decisions, deny precedence and forwarding are unchanged; only the convergence label of a plain allow changes.

Behavior

classify_decision_outcome (crates/assay-core/src/mcp/decision/outcome_convergence.rs) ignored its obligation_skipped_present argument. Every Allow without an applied or errored obligation became decision_outcome_kind: obligation_skipped with decision_origin: obligation_executor, including a plain allow that had no obligation at all. In real proxy captures every plain allow looked like this, with obligation_skipped_present: false and obligation_outcomes: null in the same record.

This PR:

  • adds DecisionOutcomeKind::PolicyAllow with origin PolicyEngine for an allow with no obligation outcome. ObligationSkipped is still used when an obligation was actually skipped.
  • mirrors the variant in the incident-package decision wire enum (decision_wire_v1.rs), so new records are admitted.
  • maps it in the replay reason (converged_policy_allow), the deny projection (not deny) and the restrictiveness rank (allow).
  • stamps new records with decision_basis_version: wave39_v2 (DECISION_BASIS_VERSION_V2). Replay therefore reports a pre-fix and a post-fix allow as reclassified, not identical.
  • leaves retained wave39_v1 records alone: they are read as stored and nothing re-derives their outcome. The checked-in golden combined_session_3345.tar (a wave39_v1 record with outcome obligation_applied) still reproduces byte-identically. Correction from review: an earlier version of this body said the golden held obligation_skipped; it holds the field obligation_skipped_present: false.
  • re-pins the reviewed producer-source digests in incident_decision_wire_parity.rs for the three changed producer files.

Breaking (Rust API): DecisionOutcomeKind is public and not #[non_exhaustive], so exhaustive matches outside the workspace must add an arm. This is semver-major for assay-core. The version bump is the owner's decision at release; this PR does not bump the version. The CHANGELOG says so under [Unreleased].

Seam for review: a policy decision typed AllowWithObligations whose executor recorded no outcome is now PolicyAllow, because the classifier sees only the outcome flags. That matches "nothing was applied, skipped or errored", but please check that you agree.

Decision record: Grok 4.7 High and Muse (muse-wonder-1.0) were consulted read-only. Both chose this fix as the next step and recommended this shape: a new variant, a basis bump, and old logs left unchanged.

Non-Claims

  • No scalar trust score or whole-action verdict
  • No generic identity, delegation, federation, HTTP/OAuth, or broad MCP scan
  • No provider-outcome, compliance, certification, partnership, or safe-agent claim

Test Evidence

RED

Commit 68d7f1b9d (tests only), run against unchanged code:

  • The classifier unit test: left: "obligation_skipped" right: "policy_allow".
  • replay_diff_contract: the same failure on the plain-allow basis.
  • incident_decision_wire_parity: decision_outcome_kind=policy_allow is rejected by both enums.
  • decision_emit_invariant: left: Some("wave39_v1") right: Some("wave39_v2").

The retained-wave39_v1 test passed as a control.

GREEN

  • cargo test -p assay-core -p assay-evidence -p assay-sim -p assay-mcp-server --no-fail-fast: all pass.
  • cargo clippy (same crates plus assay-cli, --all-targets -D warnings): clean.
  • RUSTDOCFLAGS="-D warnings" cargo doc -p assay-core -p assay-evidence --no-deps: clean.
  • fmt, plus the pre-commit and pre-push hooks.

Mutations (each restored; each caught by the intended test, not by the source-digest pin):

Mutation Caught by
Skipped flag ignored again classifies_obligation_outcomes_on_allow
Plain allow back to skipped plain_allow_without_obligation_outcomes_is_policy_allow_not_skipped
Normalization stamps wave39_v1 test_event_contains_required_fields
Wire enum lacks policy_allow nested_structs_and_all_enum_variants_match_the_producer
Replay reason text reverted basis_extraction_contains_frozen_fields
PolicyAllow ranked restrictive classify_replay_diff_stricter_and_looser

Status: parked until the 7.0.0 decision

The owner keeps this PR open and blocked until the release decision about 7.0.0. The required Semver public gate fails here by design: it reports enum_variant_added on DecisionOutcomeKind::PolicyAllow while the workspace is still 6.15.0, and that is exactly the version question the owner has reserved for the release.

Review history: Grok 4.7 High reviewed 702ef78c272c73ba5be906741c691862c1617977 and returned BLOCKED with one finding, the semver gate. Its classification checks all held. It also had two minor notes:

  • The CHANGELOG called the evidence wire enum a Rust API break, but that enum is private. Fixed in the current head.
  • This body said the golden fixture held obligation_skipped. That is corrected above.

The current head contains only the CHANGELOG change since that review, so it needs its own review before landing.

Review Quorum

  • One non-building agent review
  • Every actionable finding is fixed or has a technical disposition
  • Any delegated proof targets this exact head SHA

RED: classify_decision_outcome labels every allow without an applied or
errored obligation as obligation_skipped / obligation_executor, even
when no obligation outcome exists (obligation_skipped_present=false in
the same record). Expect a policy_allow / policy_engine outcome, a new
decision_basis_version for newly emitted records, the wire enum parity
to admit policy_allow, and retained wave39_v1 records read as stored.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…_allow

classify_decision_outcome ignored obligation_skipped_present, so every
allow without an applied or errored obligation became
obligation_skipped / obligation_executor, including plain allows with
no obligation at all. Real proxy captures showed it on every allow,
next to obligation_skipped_present=false in the same record.

Add DecisionOutcomeKind::PolicyAllow (origin PolicyEngine) for that
case and keep ObligationSkipped for an actually skipped obligation.
Mirror the variant in the incident decision wire enum, map it in the
replay, deny and restrictiveness projections, and stamp new records
with decision_basis_version wave39_v2 so replay does not compare a
pre-fix and post-fix allow as identical. Retained wave39_v1 records
are read as stored. Re-pin the reviewed producer-source digests.

Breaking for the Rust API: DecisionOutcomeKind is not non_exhaustive.

Decision: Grok 4.7 High and Muse (muse-wonder-1.0) both chose this as
the next step and recommended this shape.

…ON_V2

Drops the now-unused V1 import that clippy -D warnings rejected; the
literal wave39_v2 stays pinned in replay_diff_contract.

📝 Summary

Summary by CodeRabbit

  • Bug Fixes
    • Allows with no obligation outcome are now recorded as policy-engine decisions instead of obligation-skipped decisions.
    • Replay preserves the stored classification of older records; new records use the updated decision-basis version.
  • Changed
    • Added the PolicyAllow outcome and policy_allow incident-package value. Rust clients with exhaustive matches must handle the new outcome.

MCP decision records now classify allows without obligation outcomes as PolicyAllow from PolicyEngine. Normalized records use decision-basis version V2. Replay preserves stored V1 classifications, and the incident-package wire enum includes policy_allow.

Non-building review of head 702ef78c272c73ba5be906741c691862c1617977 (base 62f857ea0d6f41ee3b1ab324ff27bc5b41743fe9). I did not build this change and I did not author a governing spec. I was consulted on the choice of next step only.

Verdict: BLOCKED. The classification claims hold. The block is the required semver job, which has already failed on this head.

F1, open: semver job failed

Semver public gate / Wave 0 semver checks (public crates) failed on this head: https://github.com/Rul1an/assay/actions/runs/39866154935/job/120129853240 (exit 100).

Printed from that job log:

Checking assay-core v6.15.0 -> v6.15.0 (no change; assume patch)
--- failure enum_variant_added: enum variant added on exhaustive enum ---
  variant DecisionOutcomeKind:PolicyAllow in crates/assay-core/src/mcp/decision/outcome_convergence.rs:16
##[error]Process completed with exit code 100.

Workspace Cargo.toml is still version = "6.15.0". The baseline script resolves v6.15.0. .github/workflows/semver-public.yml (around the baseline comment and the check-release invocation) treats a break whose manifest version does not cover it as a failure. This diff touches crates/assay-core/ and crates/assay-evidence/, both in scripts/ci/derive-semver-published-lib-crates.py, so semver_relevant=true. The job stopped on assay-core and did not run assay-evidence.

Public API on this diff:

  • Breaking, assay-core: assay_core::mcp::decision::DecisionOutcomeKind (pub via crates/assay-core/src/mcp/mod.rs pub mod decision and decision.rs re-export) gains PolicyAllow and is not #[non_exhaustive]. Exhaustive downstream matches stop compiling. cargo-semver-checks names that enum_variant_added.
  • Additive, assay-core: DECISION_BASIS_VERSION_V2 (crates/assay-core/src/mcp/decision/replay_compat.rs:10), re-exported from decision.rs.
  • Wire behavior, assay-core: a newly emitted allow with no obligation outcome is policy_allow / policy_engine / wave39_v2 instead of obligation_skipped / obligation_executor / wave39_v1.
  • assay-evidence: DecisionOutcomeKind in crates/assay-evidence/src/incident_package/decision_wire_v1.rs:496 also gains PolicyAllow, but that module is private (incident_package/mod.rs mod decision_wire_v1, not pub mod) and nothing re-exports the type. That is not a crate-public Rust API break. It widens admission: old tags still deserialize, and policy_allow now does too. The changelog's "Breaking (Rust API)" bullet is accurate for assay-core and overstates the evidence enum.

A major bump, or a recorded semver override, is what makes this head pass that gate. Nothing else in the diff needs to change for that.

1. Consumers of the new variant

I looked for a missed match and did not find one that mishandles policy_allow.

Searches: DecisionOutcomeKind:: and ObligationSkipped / ObligationApplied / ObligationError over *.rs; obligation_skipped, policy_allow, and decision_outcome_kind over *.rs, *.py, *.json, *.yml, *.md; docs/reference; assay-python-sdk; crates/assay-mcp-server; crates/assay-cli/tests. The python SDK, docs/reference, and JSON/YAML files have no obligation_skipped and no DecisionOutcomeKind. That is "I did not find one" in those trees, not a proof that a generated file outside the repo is in sync.

Exhaustive arms that name PolicyAllow:

  • outcome_convergence.rs:80-89 produces it only when the allow has no applied, skipped, or error outcome.
  • replay_compat.rs:66-75 maps it to converged_policy_allow. The deny and obligation arms are unchanged.
  • deny_convergence.rs:60-67 puts it on the same not_deny_projection arm as the obligation kinds (outcome_not_deny). Deny arms above it are unchanged.
  • replay_diff.rs:232-235 ranks it 1, with the obligation kinds. Deny kinds stay rank 2 (:229-231).

The wildcard I expected to be the miss is crates/assay-sim/src/attacks/consumer_downgrade_next/fixtures.rs:74-79:

Some(PolicyDeny) | Some(FailClosedDeny) | Some(EnforcementDeny) => 2,
_ => 1,

PolicyAllow falls in _ and gets rank 1. That is the same rank restrictiveness_rank assigns explicitly. The comment on that function says deny is 2 and allow/obligation is 1. Not a silent deny, and not a drop.

crates/assay-cli/tests/cli_json_identities.rs does not inventory this enum. Its header says it checks assay.<segments>.vN schema identities in CLI JSON documents. A missed policy_allow would not turn that test red, and there is no row there that should have changed.

normalization.rs:14 OUTCOME_REASON_CODE_SKIPPED = "obligation_skipped" is the obligation-outcome reason code (ObligationOutcomeStatus::Skipped), a different field from decision_outcome_kind.

The two serde enums (core outcome_convergence.rs:7-17, evidence decision_wire_v1.rs:496-504) both end in PolicyAllow under rename_all = "snake_case". incident_decision_wire_parity.rs:271-281 lists policy_allow and assert_parity checks both deserializers. That test file passed: producer_source_inputs_remain_at_the_reviewed_mapping ... ok and test result: ok. 6 passed.

assay_core tests that touch the new path also passed, including plain_allow_without_obligation_outcomes_is_policy_allow_not_skipped, emission::test_policy_allow_emits_once, and retained_wave39_v1_skipped_record_keeps_its_stored_classification. Lib result: test result: ok. 1023 passed; 0 failed. cargo test -p assay-core --offline exit 0.

Forwarding does not read the outcome kind. handle_policy_decision (proxy/client/branches.rs:37-48) matches PolicyDecision. handle_allow_branch (:51-67) returns BranchOutcome::Forward and then emit_decision. Deny is the PolicyDecision::Deny arm. Those arms are not in this diff.

2. Retained package

combined_session_3345.tar does not store decision_outcome_kind: obligation_skipped. The only decision object (objects/0ba5f00a...) prints as:

decision='allow'
reason_code='E_TOOL_UNCONSTRAINED'
typed_decision='allow_with_obligations'
decision_outcome_kind='obligation_applied'
decision_origin='obligation_executor'
decision_basis_version='wave39_v1'
replay_diff_reason='converged_obligation_applied'
obligation_skipped_present=False
fulfillment_decision_path='policy_allow'

Byte counts in the tar: obligation_skipped 2 (both are the field name obligation_skipped_present), wave39_v1 2, wave39_v2 0. The assessment copy of the same record is the same shape. fulfillment_decision_path: policy_allow is the old path enum, not the new outcome kind.

Nothing re-derives that stored kind. decision_wire_v1::validate (decision_wire_v1.rs:529-531) deserializes a DecisionEvent and drops it. Incident verify calls that and keeps the original JSON (verify.rs:2314-2316 for assay.tool.decision, and :2472-2486 for proxy-decision-ndjson, which pushes value unchanged). classify_decision_outcome is not in assay-evidence. basis_from_decision_data (replay_diff.rs:101-109) copies data.decision_outcome_kind and data.decision_basis_version; a missing version still defaults to DECISION_BASIS_VERSION_V1, it does not rewrite a present one. The new unit test retained_wave39_v1_skipped_record_keeps_its_stored_classification passed.

cargo test -p assay-evidence was killed after free space fell from 7.5 GiB to 5.9 GiB (stop was 6 GiB). Before the kill, writer_reproduces_retained_container_fixtures ... ok and tests/incident_container.rs reported test result: ok. 16 passed. That test reads every *.tar in the fixture dir, including this package, and asserts the rewritten container equals the original bytes. It does not call verify_incident_package.

The already-linked incident_package_verify binary, run with cwd crates/assay-evidence, then printed test result: ok. 114 passed; 0 failed. That binary does not load combined_session_3345.tar (no test names it; read_fixture callers use other files). So I did not execute verify_incident_package on this golden. The deserialize-only path above is why an added variant cannot rewrite obligation_applied.

3. Replay bucket

A wave39_v1 plain allow and a wave39_v2 plain allow land in ReplayDiffBucket::Reclassified. That is the right bucket for this comparator.

classify_replay_diff (replay_diff.rs:172-195) returns Unchanged only when the bases are equal, then EvidenceOnly only from same_effective_decision_class (:198-224), which requires decision_outcome_kind, decision_origin, decision_basis_version, and replay_diff_reason to be equal. The v1 record is ObligationSkipped / ObligationExecutor / wave39_v1 / converged_obligation_skipped. The v2 emit is PolicyAllow / PolicyEngine / wave39_v2 / converged_policy_allow (normalization.rs:117 stamps DECISION_BASIS_VERSION_V2; reason_from_outcome_kind supplies the reason). Both ranks are 1 (:232-235), so the function does not return Stricter or Looser. It returns Reclassified.

EvidenceOnly would be wrong: those four fields are part of the effective class, not the residue (policy_version and policy_digest are what that function ignores). Unchanged would hide the label change. The changelog says the same thing: reclassified, not identical.

I did not execute classify_replay_diff on a constructed pair. A new test would have compiled, and free space was already under 6 GiB. The two halves that did run are the retained-v1 copy test and the v2 emit tests above.

4. AllowWithObligations with no outcome

Not a defect on the shipping emit path. It is a blind spot in the classifier, and the contract test locks the blind spot in on purpose.

classify_decision_outcome sees only the three presence flags. All false on an allow becomes PolicyAllow (outcome_convergence.rs:85-89). It never sees typed_decision or data.obligations.

PolicyDecision::typed_contract sets AllowWithObligations only for AllowWithWarning, and that arm always pushes one legacy_warning obligation (contracts.rs:59-62). execute_log_only (obligations.rs:21-111) returns one outcome per obligation, never an empty vec for a non-empty input. The handler assigns that result before emit (tool_call_handler/evaluate.rs:82-83). The proxy does the same (proxy/decisions.rs:109-110) and then refresh_contract_projections (:154). Those paths do not emit AllowWithObligations with an empty outcome list.

The replay fixture does. allow_event (replay_diff_contract.rs:11-24) sets typed_decision: AllowWithObligations and leaves obligations and outcomes at Default (empty), and the updated test expects policy_allow. The typed label stays on the record; only the outcome kind says plain allow. An empty obligations list is not an obligation that failed to run.

The case that would hide expected work is a non-empty obligations vec and an empty obligation_outcomes vec. refresh_fulfillment_normalization (normalization.rs:67-92) would still set all three flags false and store PolicyAllow. To tell that apart from a plain allow, the classifier has to see the obligations list (non-empty, and no outcome status), or typed_decision == AllowWithObligations together with that empty outcome list. Presence flags alone are false in both situations. I did not find a production caller that builds the non-empty-obligations / empty-outcomes pair. with_policy_context is public and would, if a caller passed it.

5. Producer-source digests

Independent SHA-256 of the file bytes matches every pin in producer_source_inputs_remain_at_the_reviewed_mapping (incident_decision_wire_parity.rs:578-611), including the three this PR re-pinned:

fe1ec8953164ba565b28953398cc2da489a669d836dfa66349c239b25ded3ae9  deny_convergence.rs  PINNED-MATCH
f14c001148949dc08087035da80fc571edc89ff5bbb44c1f2be85714149fe9db  outcome_convergence.rs  PINNED-MATCH
727f3d73bfb7873f05046c77567c8fd14af9c065ed87d2f07abf435c0d88c560  replay_compat.rs  PINNED-MATCH

The other four pins in that table (event_types.rs, policy/types.rs, consumer_contract.rs, context_contract.rs) also matched. The test that asserts them passed.

The mapping those files guard is right for this change: the enum tag policy_allow, the reason converged_policy_allow, and the not-deny projection. normalization.rs (SHA-256 c611d8d5e4cc927e8f59c815a2528748076ff5a630fd936baaa2b863be8084b9) and decision_wire_v1.rs (645a29818fd2e7937dc3452f7566ac2b6e801a1aadd5ba1fe2eff48919abed52) are not in that pin. The version stamp is guarded by the emit test, not by the digest. The evidence enum is guarded by assert_parity on policy_allow, not by a source digest. I did not mutate a pin and recompile; free space was under 6 GiB. The passing assertion plus the independent digest is the check I have.

Tests and CI

  • cargo test -p assay-core --offline with CARGO_TARGET_DIR=/Users/roelschuurkes/wt-claude-policy-allow/target and CARGO_INCREMENTAL=0: exit 0. Started at 7.5 GiB free, ended at 6.5 GiB.
  • cargo test -p assay-evidence was killed once df -h / showed 5.9 GiB free. Completed before the kill: incident container 16 passed (including the retained-tar byte round-trip); incident kernel admission's 12 tests each printed ok and the process died before cargo's summary line. Afterwards, the already-linked incident_package_verify binary from the crate directory: 114 passed; 0 failed. A first run from the repo root failed on relative fixture paths (No such file or directory); that was the working directory, not the suite.
  • cargo test -p assay-sim was not started. Disk was already under 6 GiB. The builder's already-linked lib test binary assay_sim-37128a49a8423d98 (00:17 local, and strings finds policy_allow) printed test result: ok. 45 passed, including tests::test_quick_suite. assay-sim has no tests/ integration crate. I did not recompile it.

Latest gh pr checks 3406 non-passing contexts:

  • Semver public gate / Wave 0 semver checks (public crates): fail. Inside this review. This is F1.
  • review-record-check: fail. Outside the product change. It is the missing record on this head; this comment is that record.
  • Build + Test (ubuntu-24.04): pending. Outside this review. I did not inspect that job.

git status --short on the review worktree was empty at the start and after these runs. No source file was left modified.

Review of the previous head: the enum in assay-evidence's
decision_wire_v1 is private, so adding policy_allow widens incident
admission without breaking that crate's public API. The assay-core
break stays as stated.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win

Bump the shared release line to 7.0.0.

DecisionOutcomeKind is a public exhaustive enum. Adding PolicyAllow is a semver-major change. The workspace remains at 6.15.0, so the required semver gate can fail.

The only documented exception is a workflow-dispatch override_reason. No override applies to this PR. Update the shared version and all internal 6.15.0 dependency pins before merge.

The earlier CI failure was on the parent commit, not a current-head execution. The current-head failure is the consequence of the unchanged breaking enum and release configuration.
