Follow-up to GoogleCloudPlatform#6746. Adds the upgrade path — Phase 2 of concurrent_node_pool_ops plus pipelined mode and the --k8s_mgmt_target_version / --k8s_mgmt_pipeline_scenario_a flags — on top of the create/delete split. Stacks on GoogleCloudPlatform#6746; will re-point base to master once that merges
