Problem

Production Responses WebSocket sessions were dying mid-conversation: AxonHub's upstream socket became unwritable (write tcp ...: write: connection timed out) and the downstream client saw close 1006 (abnormal closure).

The first revision of this PR added an internal recovery that dialed a replacement upstream connection and resent the original response.create on a write failure. That approach is unsound and has been removed.

Why resending is unsafe

WriteJSON returning an error does not prove the request was never delivered. The upstream may have accepted and already executed response.create, so resending the same payload on a fresh connection can execute the request twice — duplicate side effects, duplicate tool calls, duplicate billing. The connection also carries session state (lastResponseID, generation, retained input), so a replacement connection is not equivalent to the one that failed.

What this PR does instead

A failed session is evicted and the failure is surfaced instead of being replayed. A replacement upstream connection is established only where the payload was provably never written:

  • a reused session whose prepared payload requires a fresh connection — an append-only delta that cannot be expressed, a non-native payload, or an explicit previous_response_id that does not belong to the tracked session
  • a pooled session that is already closed, expired, or generation-mismatched

Both cases happen before any write on that connection, so the first write on the new connection is not a replay.

Supporting fixes kept from the original revision: webSocketStream.finish reads the lease under the lock (reading s.lease outside it races with a concurrent lease handoff); webSocketStream.setErr keeps context cancellation authoritative unless a terminal event was already observed; webSocketLease.write aborts a stuck write when the context finishes instead of waiting for the full 30s write deadline.

llm/pipeline/pipeline.go and llm/transformer/shared/responses_continuation.go are back to their previous behaviour, since the reconnect sentinel is no longer needed.

Scope

The recovery is keyed on the upstream transport only, so it applies to both downstream protocols:

downstream upstream affected
WebSocket WebSocket yes
HTTP/SSE WebSocket yes
HTTP/SSE HTTP/SSE no
WebSocket HTTP/SSE no

Verification

cd llm && go test -race -shuffle=on -count=1 ./... passes, gofmt and git diff --check are clean, and TestWebSocketExecutorDoesNotReplayAfterReusedSessionFailure pins the invariant: after a reused session is aborted by the peer, the next turn must fail and the upstream connection count must stay at 1 (the previous behaviour reached 2).

Not included

The upstream long-connection timeout (context deadline / peer write timeout while a socket is blocked) is a separate concern and is unchanged here.

Summary by CodeRabbit

  • Bug Fixes
    • Improved handling of canceled or explicitly closed WebSocket streams so they stop promptly without reporting spurious read errors.
    • WebSocket writes now stop when the request context is canceled, and cancellation errors are reported when cancellation occurs before a terminal event.
    • Failed request submissions and errors from reused connections are returned without automatically opening a replacement connection or replaying the request, avoiding duplicate submissions and making failures clearer.

Reconnect a reused upstream socket once before any response event when the request contains full input. Stop pipeline retries after reconnect exhaustion so a failed recovery cannot replay the request a third time.

📝 Walkthrough

The changes update WebSocket stream cancellation and closure handling. They also document and test that failed requests are surfaced without replaying them on a fresh connection.

This change makes failed upstream WebSocket sessions return an error instead of silently resending a request that may already have run. It also tightens handling of stream cancellation and closure. Tests cover the no-replay behavior. No outstanding issues block merging.

[High impact] The PR appears safe to merge; no actionable new issue was found.

Summary

The executor surfaces failed WebSocket writes without opening a replacement connection or resending the request.

  • Failed WebSocket writes now return errors without resending requests.
  • WebSocket streams stop cleanly when callers close or cancel them.

The two previous threads are unnumbered and resolved. Their replacement-request scenarios no longer apply because the internal replay path has been removed.

Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.

Remove the duplicate lease.release(evict) call.

finish calls lease.release(evict) on Line 1301 and again on Line 1302. sync.Once makes the second call a no-op, so behavior does not change. The second call looks like a leftover from the removed reconnect handoff, where the lease could change between calls. Delete one of the calls.

1394-1408: 🎯 Functional Correctness | 🔵 Trivial | ⚡ Quick win

Cover the reused-session failed-write branch deterministically.

SetLinger(0) and the fixed sleep do not guarantee that the client has processed the reset before WriteJSON. The second DoStream can therefore succeed and fail only in Next, which is a valid read-error case but does not exercise the failed-write path.

Close the pooled client's conn while leaving pooledWebSocketConn.closed unchanged, then assert the immediate write error. Keep the existing read-error coverage in a separate test if needed. This requires only a small test change and protects the no-replay behavior.

A write failure on a reused Responses WebSocket session is ambiguous: the
upstream may already have accepted and executed response.create. Resending the
same payload on a replacement connection can therefore execute the request
twice, with duplicate side effects and billing.

Evict the stale session and return the error instead. A replacement connection
is now established only where the payload was provably never written:

- a reused session whose prepared payload needs a fresh connection (append-only
delta, non-native payload, or an explicit previous_response_id that does not
  belong to the tracked session)
- a pooled session that is already closed, expired, or generation-mismatched

This recovery is independent of the downstream protocol, so it applies to
http-to-websocket routing as well as websocket-to-websocket.

The reconnect sentinel is no longer needed by the pipeline, so
llm/pipeline/pipeline.go and llm/transformer/shared/responses_continuation.go
return to their previous behaviour.
