NEXO is a daily clinical reasoning game. One fictional patient case, six clues revealed in order, and feedback based on how close your answer sits to the correct ICD-10 code.

Every case in NEXO is fictional and written for teaching. NEXO is not medical advice and is not for use with real patients.
NEXO is a medical education product for practising diagnostic reasoning. A case opens with a fictional patient. You reveal clues one at a time and submit a diagnosis whenever you are ready. The fewer clues you need, the better the result. It is designed for a few minutes of daily practice rather than long study sessions.
A case carries up to six progressive clues: presentation, history, physical exam, labs, imaging, and whatever else the case calls for. Multimedia cases add the finding itself, such as an image, a tracing, or an auscultation.
When you answer, NEXO does not only mark it right or wrong. It compares your diagnosis to the correct ICD-10 code and reports the distance between them.
| Feedback | Meaning |
|---|---|
| Exact ICD | You hit the correct answer. |
| Same category | Very close to the correct ICD. |
| Same group | Good direction, still broad. |
| Same chapter | Related area, but still far. |
| Different chapter | Your hypothesis is far from the answer. |
That gradient is the point of the game. Being wrong within the same category is a different kind of wrong from being wrong in another chapter, and the feedback tells you which one you were.
NEXO is an educational simulation. It is not clinical decision support, not a symptom checker, and not a patient-facing product. It holds no real patient data. Every case is written for teaching and reviewed before it reaches players.
Medical students, residents, and physicians who want short, repeatable practice at building a differential and committing to a diagnosis under incomplete information.
A generic quiz app scores an answer as right or wrong. NEXO scores how far off you were, using the ICD-10 hierarchy as the ruler. Two people can both miss a case and learn different things, because one was in the neighbouring category and the other was in a different chapter entirely.
The second difference is the shape of a case. Clues arrive in sequence, so the cost of guessing early is real, and committing late with more information is not free either.
NEXO covers hundreds of fictional clinical cases, grouped by medical specialty, in daily, continent, and multimedia formats. The full list of diagnoses is published and kept current at nexo.wiki.br/en/collection.
No counts appear in this README on purpose. The library grows, and that page is the only place that stays correct.

NEXO is published in Portuguese, English, Spanish, French, German, and Russian. Clinical text is translated and reviewed by native speakers before publication.
Every case is written against a fixed editorial contract before it reaches players. A case carries a diagnosis, its ICD-10 code, a specialty, the progressive clues, and an explanation that names the plausible differentials and says why each one is excluded. Clinical content is not published without review.
- Game logic runs on the server. The client never decides whether an answer is correct, so a result cannot be altered from the device.
- ICD-10 proximity is derived from the structure of the code itself, walking up from the code to its category, group, and chapter. It does not compare diagnosis names as text.
- Content ships in six languages through a pipeline where a native speaker reviews the clinical text before it goes live.
- Public counts are read from the database at request time. Nothing in the product hardcodes how many cases exist, which is why this README links to the case library instead of quoting a number.
